QED POWER FORWARD SCHOLARSHIP CONTEST

AS A CONDITION OF RECEIVING A SCHOLARSHIP, STUDENTS 18 OR OLDER MUST SIGN THIS FORM. IF THE STUDENT IS UNDER THE AGE OF 18, THIS FORM MUST BE SIGNED BY A PARENT OR LEGAL GUARDIAN. THIS CONSENT AND RELEASE FORM, TOGETHER WITH THE OFFICIAL CONTEST RULES, ARE A BINDING AGREEMENT. A STUDENT IS NOT ELIGIBLE TO BE DECLARED A WINNER UNTIL THIS FORM IS SIGNED AND RETURNED TO SONEPAR MOUNTAIN HOLDINGS, LLC, D/B/A QED (“SPONSOR”). THE STUDENT IDENTIFIED AT THE BOTTOM OF THIS FORM WILL BE REFERRED TO AS “THE STUDENT.”


  1. I have read and understand the Official Contest Rules located at Power Forward Scholarship Rules | Quality Electrical Distribution (QED), which are binding on the Student.

  2. I acknowledge that the Official Contest Rules contain, without limitation, a release of liability and a license that allows Sponsor to use the essay created by the Student without any additional notice or financial compensation.

  3. I authorize Sponsor to publish information concerning the Student, including the Student’s full name, age, school, grade level, city, state, and image, without any additional notice or financial compensation. I further represent that I have read, understand, and agree to Sponsor’s Privacy Policy.

  4. I agree to provide any additional information or to execute any additional form(s) that are necessary for the Student to claim a scholarship.

  5. I represent that the Student is enrolled at IECRM Independent Electrical Contractors Rocky Mountain for the 2026 Fall semester. I further represent that any scholarship the Student receives from Sponsor will be used to pay for books and fees required for courses at the educational institution.

  6. (6) Parents and legal guardians signing this form represent and warrant that: (a) the Student meets all eligibility requirements listed in the Official Contest Rules; and (b) they give full authority to give consent, to release, and to contract for the Student.
